The strength of TCS is our commitment to individuals and their families through an innovative, creative and flexible approach to community care. We have achieved success in the social services sector for more than 30 years by pursuing the goals of meeting and exceeding the expectations of individuals in our care and their families. We are dedicated to helping each individual live a meaningful life within the community. TCS’s staff members know that needs change over time so we have the experience – and resources – to develop flexible, individualized plans that offer care with dignity and help individuals and their families attain their goals. With each passing year our pursuit of innovation allows us develop new tools and strategies to be even more effective.
Thompson Community Services utilizes a person centred planning process for all individuals that we serve. The persons served are involved in all aspects of this process. Individualized plans are completed annually and reviewed minimally every three months. The plan encompasses the individual’s physical, medical, psychological, social and spiritual needs.
Individuals and their support network are consulted to develop personal goal statements for each person. This results in a set of outcomes to assist the individual in attaining skills and achieving personal growth. As a component of the Person Centred Planning process, we establish outcomes each year that are applied at an organizational, program and individual level.
Action plans are developed to assist in achieving the desired goals. Data is collected to determine if progress is being realized in the attainment of goals. To ensure accountability and to assist us in the process of data collection, we invite the opinions of stakeholders with regular surveys, including a satisfaction survey, service expectation survey, and assessments upon admission and discharge. The results are reviewed regularly by the team.
Each individualized plan corresponds directly with agency philosophies and outcome statements. By analyzing our success with attaining the individual’s goals, we are able to determine if the agency is achieving its goal. This process results in an integrated system that links our philosophical approach directly to individual goals and action plans, individual outcomes and feedback from major stakeholders.

Vocational opportunities that may include work in the community such as lawn maintenance, landscaping, paper routes, recycling programs, meal delivery to seniors, housekeeping, pet walking and community beautification.

Person Centered Planning
TCS has integrated the Dr. Schalock’s Quality of Life domains for the purpose of enhancing the quality of life of the people that we support. We focus on each individual’s personal support plan in order to achieve his/her desired quality of life outcomes. Each individual’s plan is monitored, tracked and evaluated to ensure effective delivery towards personal goals and outcomes. All health care, behavioural support and safety plans are updated and monitored for ongoing effectiveness in achieving personal quality of life outcomes.

Individuals are assessed in terms of risk to self or others and in areas of vulnerability according to the CLBC Behaviour Support and Safety Planning policy. These areas are captured on written approaches and risk assessments that are reviewed and approved by the TCS senior clinical team and CLBC. Health care plans, behaviour and safety plans are developed for those individuals who require them. An individual personal profile is maintained with each individual that includes diagnosis, communication issues, risks, family, advocates and community likes and dislikes.
Thompson Community Services welcomes and encourages the input and involvement of families and support networks into every aspect of their loved ones lives. The families and supports of individuals are regarded as experts regarding their loved one and their involvement and inclusion is always appreciated and supported.
Our service focuses on a partnership model with families and support networks. This means a recognition that families are very often more knowledgeable and concerned about their loved ones than caregivers or professionals. It is a continuing goal of all of our programs to engage and include families.

Prevention
An important concept underlying the programs of TCS is that of prevention. The ongoing support and the development of a support system sets the stage for individuals to more effectively manage crisis as it arises. It has been our experience that as relationships develop with program staff and as a support system is established, the number of crisis situations decreases. Our model creates the supports, the ongoing relationships and the communication patterns that combine to minimize crisis.
Our agency strives to prevent hospitalization by implementing a proactive clinical plan for each person. Through consistently scheduling medication reviews, regular blood work, and medical/ psychiatric assessments, we are very diligent in promoting health and wellness, often preventing a crisis from occurring.

We then begin to develop a support plan with the team that is engaged with the individual. Staff members receive training to ensure that they are able to provide the necessary support to each individual.
The plan is then implemented, closely monitored and reviewed to ensure its effectiveness. Plans may be modified and adjusted as individual needs change over time.
Clinical Services Consultant
We involve our Clinical Services Consultant to review all health related protocols and safety plans and to ensure that all necessary medical and psychiatric services are being accessed, in order to promote wellbeing and prevent crises. As well, the Clinical Services Consultant will liaise, where appropriate, with community professionals including physicians and other health professionals.
